This is an original unused NYC bus operator form, not a reproduction. These sheets were used by bus operators to record information from their day's work, including route and run information, bus and fare box numbers, trip times, fare-box readings and passenger/fare information.
The front includes fields for:
- Depot, route number and run number
- Operator name, pass number and badge
- Report and clear times
- Bus and fare box numbers
- Cash, token and penny fare-box readings
- Add-A-Ride tickets
- Free transfers and various passenger/pass categories
- Trip and point-leaving information

The reverse contains a period instruction for operators to record the number of senior citizens and handicapped passengers riding under the extended half-fare program during designated morning and afternoon hours.
The printer/form code at the bottom reads 58-61-2074- 700M 6/82, dating this example to circa June 1982.
